Insurance & Benefits Verification
Please call the member services number on the back of your insurance card before our first appointment to verify your benefits. Ask the representative to confirm if "David McDaniel" or "David McDaniel Counseling" is currently an in-network provider for your specific plan.
Insurance Accepted
I'm an active provider for Blue Cross Blue Shield (PPO, Blue Choice, and EPO/HPN), Aetna, and WebTPA. Credentialing for Cigna and Evernorth is currently pending.
If you have the UHC StudentResources plan as a student at the University of Illinois, you're also covered. You simply pay your standard co-insurance at the time of your session, and the plan takes care of the rest.
Insurance Not Accepted
I'm not a provider for standard UnitedHealthcare/Optum commercial plans, HMO, Medicare, or Medicaid (Managed Care) plans.
Out-of-Network Coverage & Billing
If an insurance provider is not listed above, or if my credentialing with a plan is still pending, I'm considered an Out-of-Network Provider and clients are responsible for the full professional fee at the time of our session. Since many PPO plans offer out-of-network benefits, I can provide a monthly "superbill" (an itemized medical receipt) for clients to submit directly to their insurance company. Depending on the plan's specific out-of-network coverage, they may reimburse the client for a portion of the cost.
Important Note: Due to a mandatory contract transition period, I'm legally unable to offer out-of-network superbills or cash-pay services for standard UnitedHealthcare or Optum members at this time.
Billing and Fees
Standard Rates (billed to Insurance)

Initial Intake Session: $265

Individual Therapy Session (53+ min): $215
Note: If you're using insurance, you're typically responsible only for your specific co-pay and deductible. These rates are adjusted based on my contract with your health plan.
Reduced Rates & Sliding Scale
I believe therapy should be accessible. I reserve a limited number of spots for clients who face financial hardship and may need a reduced rate. Please ask about availability during our initial consultation.
Good Faith Estimate
Under the federal No Surprises Act, you're entitled to a Good Faith Estimate of expected charges if you are not using insurance. The GFE outlines the maximum expected cost of non-emergency services during a specified timeframe and is included in your intake forms.
Payment Methods
Payment is due at the time of service. I accept Mastercard and Visa, as well as HSA and FSA cards.
Cancellation Policy
Please provide at least 24 hours' notice if you need to cancel or reschedule. Missed appointments or cancellations made with less than 24 hours' notice will be charged a $100 fee. Insurance companies do not cover cancellation fees. These are billed directly to you.
